Access to my information

In accordance with the Health Records Act 2001 patients can request to access information from a medical record.

Once your application form has been received, a search will be conducted for the information you seek.

Under the Health Records Act 2001 Masada Private Hospital has 45 days to provide the requested information.

There are costs involved in providing this information and an invoice will be sent once an initial request is made. These are in accordance with the Health Record Regulations 2023.

Registered Post $12.00
Photocopying 0.20c per page
Scanning 0.10c per page
Retrieval from secondary storage $19.60
